7 Milestone 1.1 The Participant must complete and sign the HIN “initiative” contract as a selected participant in this SIM initiative. Next Step: Please contact HIN with any questions in order to complete this process. Deadline: 5/9/2014 7

8 Milestone 1.2 Complete HIN’s Bidirectional Participant Agreement for HIE services in order to receive the initial $10,000 payment. –From contract: “Participating in this initiative waives all of HIN’s standard HIE participant fees for the life of the grant. At which time the grant ends, the participant is not required to continue to participate with HIN, however the hope is to prove the value of such participation and continue the service relationship.” Next Step: Please contact HIN with any questions in order to move forward with this process. Deadline: 7/30/2014 8

9 Milestone 1.3 Confirm the staff and client HIE participation and consent education plan and timelines with HIN staff. Next steps: Attend the June webinar; review communications materials sent in advance of the June meeting to prepare questions. Consider implementing the staff education process earlier than November in order to access the “Portal” prior to milestone 2 HIN will be reaching out to meet with current active HIE user teams in this initiative to obtain specific feedback on your experience. Deadline: 9/30/2014 9

10 Milestone 1.4 Confirm EHR’s technical capabilities: 1.Interfaces are installed and ready to test 2.VPN connection in place over summer 3.* Begin viewing the HIE Portal prior to milestone 2 is an ideal option* 4.HL7 interoperability standards for: -Admission, Discharge, Transfer messages [ADT]; -Continuity of Care Document [CCD/CDA] -Data categories examples: Diagnosis codes, procedures codes. (see next slide for next steps….) Deadline: 9/30/2014 10

11 Milestone 1.4 - continued Next Steps: 1.We will schedule one phone meeting with you by June 30 th to begin a technical assessment with your IT leadership and EHR vendor (if you have one). –We will reach out to you via email this week offering times to meet with your organization and your EHR vendor –Assessment forms will be sent with this email 11

12 Milestone 2.1 Complete HIN’s “onboarding” and education processes with staff for HIE connection and participation, including legal rules for “opt-in” “opt-out” client consent education. Deadline: 11/30/2014 12

13 Milestone 2.2 HL7 Data Integration Testing Completed for ADT and CCD messages: –Complete data validation scenarios in the HealthInfoNet Test environment. This involves creating a Test patient in your EHR, updating the record according to the data validation scenario, and verifying that the data has populated correctly within the HealthInfoNet Test environment. Completing the above step in HealthInfoNet Test will prepare for the Milestone 3 requirement of quality measurement via the HIE with the following Milestone 2 obligations: –Test initial HL-7 data (unique to each organization) –Test remaining HL-7 integration capabilities (for example, diagnosis codes, problem lists, medications, ORU [Unsolicited Transmission of Observation Message such as transcribed documents, test results, office notes]) Deadline: 4/1/2015 13

14 Milestone 2.3 HIE Bidirectional Connection Go-Live: –Complete data validation scenarios in the HealthInfoNet Production environment. This involves creating a Test patient in your EHR, updating the record according to the data validation scenario, and verifying that the data has populated correctly within the HealthInfoNet Production environment. –Complete executive sign off sheet. 15 Milestone 3.1 - Quality Measurement Integrate data with the HIE that can be used for quality measurement and reporting for a minimum of one measure. SIM alignment for us to assess: –What do you already measure? –Clinical measures required under Health Homes –BH Pathways to Excellence measure (initial pilot to be implemented 1/2015) –Clinical data used for State required data reporting required by DHHS (APS, EIS?) Deadline: 3/1/2016 15

16 Introducing: Quality Measurement Phases You will be learning about the data sets that are able to be integrated into the HIE. This will prepare you for your first phase in quality measurement. Phase 1: Assessment –What data you will be sending to HIN’s HIE? –What of this data do you currently measure? –What of this data could you use to meet the needs of your organization? –Do you want to and/or can you change what data you send to HIN based on this assessment? Phase 2: Selection and Testing –Select measures to test (2-4) –Work with HIN staff to determine measure specifications –HIN implementation –Testing & QA –Report findings and recommendation for implementation Phase 3: Implementation 16

17 FYI: Pathways to Excellence (PTE) BH Committee Discussion…. Public reporting measure criteria ideas discussed at Pathways to Excellence, Behavioral Health Steering Committee meeting held on April 23, 2014. Measure ideas discussed indicated that people want the measure(s) to be: free/public domain minimal/no cost training already existing (or required) (don’t create something) not tied to funding Some want a qualitative measure that captures client perspective. Qualitative info may be available in an existing Likert scale assessment and something like that would pair nicely with a more standardized measure administered by the clinician. 17

18 Milestone 3.2- Be Involved! Participate in: –future workgroup that will shape the scope and implementation for Behavioral Health quality measures and reporting processes with HIN and/or; –within the scope of the SIM projects –Other? What else are you doing we should know? 18

20 Initiative Reporting Organizations are required to report to HIN on project progress and milestone achievement: –Short 2-page report format (see meeting materials “draft report”). –Finished reports are due a week after the quarter/milestone ends. –Finished reports will be sent to Katelyn Michaud (kmichaud@hinfonet.org) 20
