When do we issue; which one?

4 When to issue a CG-835 The policy for this is: D8(dp) Policy Ltr 01-09
If the vessel is has minor damage, a route of (lakes, Bays, and Sounds or lesser) and their transit will not take them outside of D8, you can issue a CG-835. Minor damage as described in D8(dp) Policy Letter : (1) Damage to the outer hull plating, either above or below the waterline, that does not exceed either 12 inches X ¼ inch or 3 square inches; and (2) Damage that does not result in a cumulative total of more than 1 temporary repair per void or 3 temporary repairs on a barge; and (3) Damage that does not involve a cargo tank. The permanent repairs shall be commenced at the first opportunity, but not later than 45 days after the minor damage is discovered.

6 When may a PTP be issued? Reference: 46 CFR ; It basically states that; The OCMI may issue a permit to proceed to another port for repair if in OCMI’s judgment it can be done safely. There must be a written request. The permit must state the conditions under which the permit was issued and whether or not the vessel is permitted to carry freight (cargo) or passengers. The permit must be carried in the same manner as the COI. A PTP may be issued: The vessel has damage to the cargo tank. The damage is outside the scope of “minor damage”. The vessel will travel outside of D8.

CARGO (ES) CARRIED: Lube Oil VESSEL CHARACTERISTICS: Double sided, double bottom, sub chapter “OD” cargoes TRANSIT PROPOSAL: Allow transit from Mile 50 on the Upper Mississippi River to Mile 287 on the Illinois River (Joliet, IL) to offload. Contact the local OCMI upon arrival at destination. ETA TO OFFLOAD: 8 days ETA TO REPAIR FACILITY: 30 days MITIGATION MEASURES: (Describe temporary repairs) 6” vertical fracture from the deck knuckle to the rub bar and 7’x3’x5” inset all located on the starboard side shell 40’ forward of the stern. Epoxy in place over the fracture.

9 Procedure(how to) When you get the call that a Tank Barge(red flag) has damage; you need to know: Where the damage is located on the barge, (rake, wing void, cargo tank , draft mark, etc) What type of damage, (crack, dent, hole, etc) Above or below the water-line, (taking on water?) Any temp repairs? (if so what type) What is the cargo? (proper name) Where do they plan on transiting? (outside of D8?) Has a surveyor and marine chemist been onboard? (MC cert)

Then you: Determine if you need to issue a CG-835 or PTP, using the info you have collected. Notify the Prevention’s Chief of your recommendation. Notify the Command. Notify SOHV’s watch center. If your recommendation is a PTP: Then you need from the owners rep: A PTP request, you may have to one to them. Make sure all of the info is filled out. If they have pictures have them them to you. You will have to fill out the Western Rivers Notification, and the PTP(form CG-948)

Then you: Get the CO to sign the PTP. Make 3 copies of the signed PTP. Scan the signed PTP into the PTP folder in the all hands folder(preventions). Notify the Sector/Sectors zone that the vessel will transit. the PTP, WRN and the PTP request with a simple explanation of the damage and when and where the vessel will transit. Take one of the copies and tape it to the outside of a envelope, write “TO be opened by the Coast Guard only”. Put the original PTP and COI in the envelope and seal. Give a copy to the owners rep. Keep the last copy for our records. The PTP MISLE activity is separate from the IO case. That will be it’s own training. Come see the Inspection shop. As you can see this takes awhile to get through the system, let the owners rep know up front that this is not a quick turn around.
