LADDER QUIZ SB Workshop

FALL FROM HEIGHTS MOBILE LADDER

Reasons for falling off The most frequent reason people fall off ladders is: A.They mis-step, slip or lose balance B.They overreach or stand on the top rung C. The ladder breaks D. The ladder moves

Reasons for falling off D is the commonest reason for people falling off ladders: A. They mis-step, slip or lose balance (31%) B. They overreach or stand on the top rung (5%) C. The ladder breaks (4%) D. The ladder moves (48%)

Slippery! Which slippery condition occurs most often with falling off a ladder : A. The user is wearing slippery shoes B. The anti-slip of the ladder itself is no good C. The surface the user puts the ladder on is wet or slippery

Slippery! C.occurs most often with falling off a ladder A. The user is wearing slippery shoes (2%) B. The anti-slip of the ladder itself is no good (11%) C. The surface the user puts the ladder on is wet or slippery (20%)

Wrong placement of ladder In 20% of accidents users were not placing the ladder correctly (e.g. wrong angle) even though they could have. The main reason was: A. Not motivated to do so B. Inadequate procedures or planning C. Incompetence

Wrong placement of ladder A. is the main reason users were not placing the ladder correctly (e.g. wrong angle) The main reason was: A. Not being motivated to do so (57%) B. Inadequate procedures or planning (4%) C. Incompetence (12%)

On the way down When a person falls off a ladder, the chance of there being an object in the line of fall is: A 1 in 500 falls B 1 in 50 falls C 1in 5 falls D 1 in 2 falls

On the way down C. Is the chance of there being an object in the line of fall A 1 in 500 falls B 1 in 50 falls C 1in 5 falls D 1 in 2 falls

SOME ACCIDENT FACTS AND FIGURES (GISAI reportable accidents 1998-2004 analysed in STORYBUILDER)

GISAI Data There are approximately 12500 accident cases with an accident/ penalty[1] report reported between January 1998 and February 2004.[1] This is an underestimate of the total number of work related accidents as many go unreported [1][1] Sometimes referred to as prosecution- reports

Accident frequency per year In the 6 year period analysed there were per year on average: –425 contact with moving part of a fixed machine accidents –220 hit by falling objects of which 169 not crane related, 51 crane related –195 contact with flying/ejected objects –178 fall from height roof/floor/platform –173 ladder fall accidents of which 122 mobile/placement ladders –89 struck by moving vehicle –86 fall from scaffold accidents –63 Loss of containment accidents of which 40 from normally closed containments –31 contact with electricity

Deaths 1 Permanent injuries 6 Recoverable injuries 22 1.1.1. Ladder accidents (reported and investigated) Every year there are on average 4 deaths from falls from a ladder, 22 permanent injuries and 88 recoverable serious injuries

1.1.1.1 Mobile ladder accidents 67% LADDER STABILITY fails: –Mostly due to setting it up incorrectly –The rest due to an unstable surface being hit by something poor position of the person on the ladder 29% USER ABILITY fails –mostly due to loss of control over body movement such as due to the force of using a tool or a misstep 4% LADDER STRENGTH fails –Due to substandard condition or type of ladder The three mutually exclusive barrier failure types that directly led to falls:-

1.1.1 Ladders - MOTIVATION In 50% of ladder accidents, lack of motivation for safety played a role, so LACK OF: –Alertness, care & attention, –Concern for safety of self & others, –Risk avoidance, –Willingness to learn & improve

1.1.1. Ladders - CONFLICTS Conflicts with ‘profit’ goals played a role in 20% of ladder accidents ….in around 200 accidents, barriers were compromised in association with safety- profit conflicts e.g. 83 cases of conflicts with correct placement of the ladder

1.1.1 Ladders- MISMATCH In almost 50% of the ladder accidents, mismatch between the activity being undertaken and the tool or method used played a part. e.g. the ladder was too short Arbobesluit art. 7.3 lid1&3 en Art.3.16 lid1.
