1 © 2005 National Remodelling Team Purpose / rationale of the Targeted Youth Support (TYS) Project The aim of the project is to develop a workable local model for the TYS service in order to deliver improved, user-friendly services for young people with additional needs. Multi-agency TYS services will operate as a part of the integrated youth support service provided by the Children’s Trust, and will include a range of practitioners with complementary skills from different professions. The team will have a clear remit to work with mainstream and specialist services to: Provide ‘wrap around’ support, via a lead professional, for individual young people who have multiple needs Deliver effective preventative work for groups of vulnerable young people Provide an outreach, support and training role for practitioners within mainstream and more specialist services Operate within an agreed approach to assessment and referral Make use of developing systems for client records, information sharing and tracking

2 © 2005 National Remodelling Team The Scope of the Project Geographic target: Bolsover, Staveley and Brimington There is strong partnership working already seen in the Bolsover/Staveley Learning Community, the Staveley Neighbourhood Management Project and the Bolsover Local Strategic Partnership These bodies bring together five secondary schools, a special school and FE college working with multiple agencies Young people target: 13 – 19 year olds Specific issues: To develop approach covering all targeted youth issues within a framework of support for all young people in the area To map and plan the availability of a directory of service and support for young people in the area

3 © 2005 National Remodelling Team Methodology The process follows the National Remodelling Teams Change Management Model, which was used in School Workforce Remodelling: Mobilise: identify who is involved, such as partners, providers and stakeholders Discover: research current provision, what works, what do people think Deepen: Develop a deeper understanding of the issues find out why Develop: Consider models and recommend solutions Deliver: Implement the proposals
