TOBACCO FREE NORTHEAST PA FY 2011 SUCCESS Patient with Diabetes, Peripheral Artery Disease, Chronic Obstructive Pulmonary Disease and Hypertension progressed from a contemplative smoker to a non-smoker utilizing effective cognitive behavioral change, NRT and counseling.
