4 Your “Community” For purposes of this HIT Toolkit for Small and Rural Communities, “community” is defined as organizations that are legally related to one another within what is often referred to as a healthcare delivery system or healthcare delivery network Your “community” may include a hospital, physician practices that the hospital owns, a long term care facility, assisted living facilities, home health agency, DME provider, ambulance service, and other facilities under common ownership Your acquisition of this HIT Toolkit is your license to use the tools within your “community” 4
