2 Asthma Education Plan Q: How can I use the Get Well Network (GWN) in this plan? A: Prescribe Asthma Videos:  Upon pt admission to floor {new diagnosis and return admissions}  Before asthma educator visits with patient  For asthma patients only. *For RSV pts prescribe – Understanding RSV

3 1.) On Meditech, click globe with ‘R’ 2.) Select GWN Management Console Q: How do I access the GWN? A: Follow these simple steps! Asthma Education Plan Get Well Network

4 3.) Log in using same username and password that you currently use to access your computer

6 Asthma Education Plan Get Well Network 5.) Select pt by First & Last Name or Unit & Room number 6.) After pt selection, click SEARCH

7 Asthma Education Plan Get Well Network 7.) Select white paper icon 8.) Click [Show Order Set Items]] for ‘Asthma’ to prescribe videos 9.) Select SAVE EDUCATION ORDER to complete process

8 Asthma Education Plan Get Well Network Date, time, & education ordered Status indicates if video was completed, stopped, or not started Note: This screen will show any test completion, score, and missed questions. This information is helpful when educating pt/parents.

10 Asthma Education Plan Commonly Prescribed Videos New Dx of Asthma  What is Asthma  Asthma Medicines  Handling Asthma Flare-Ups  Creating an Asthma-Safe Home  Asthma Story [choose one] o Jordan {older teen} o Monica {17 yr old} o Alexis {12 yr old}  Asthma: Parents Talk  Asthma Action Plan  Nebulizers & Inhalers (if applicable)  Using a Peak Flow Meter (if applicable) Re-admits  Handling Asthma Flare-Ups  Asthma Medicines  Creating an Asthma-Safe Home  Nebulizers & Inhalers (if applicable)  Using a Peak Flow Meter (if applicable) *** Nurses can prescribe other videos at their discretion.

11 Asthma Education Plan Reassessment After pt/family video completion:  Reassess to ensure understanding of teaching:  “Teach Back”- Ask parent a scenario based question. Example: You will be working late today and Jane is going to Grandma’s house. Please tell me what you would tell Grandma to do if Jane has an asthma attack.  “Ask me three” - Verify level of understanding by focusing on the following teaching points. (*Avoid asking parent ‘do you understand’?)  What does the parent need to know about asthma?  What does the parent need to do to care for the child with asthma?  Why is this information important?

12 Customer Service Hint When assessing level of understanding, it’s important to help the parent feel comfortable. Consider using the following approach: “I want to make sure that I have done a good job teaching you what to do when you go home. Will you tell me how you will provide instructions to the babysitter about this asthma medicine? How will you explain how to use this inhaler? Remember… The Get Well Network is an adjunct for patient education. It does not take the place of reassessment.

13 Documentation We have many opportunities to improve charting about asthma and the home management plan of care. Specifics are important! The following slides show examples of good documentation.

16 Discharge Planning Remember to add specific instructions to the nursing discharge record. Include the following information:  Avoidance of asthma triggers  Follow-up information: Doctor’s name, phone #, arrangements for care (appt info)  When to take action and specific steps to take  Contact information for who to call if an asthma attack occurs or is about to occur  Information on appropriate use of controllers and relievers (medicines)
