1 Burnt Chimney Elementary School “D-E-T-E-R-M” PLAN

2 Determine (What is our goal?) In Spring of 2007, 50% of 5 th graders and 30% of 4 th graders at Burnt Chimney Elementary School will participate in their IEP meeting as measured by the IEP Participation Continuum Checklist

3 Determine (What is our goal?) In Spring of 2007, 1 5 th grader and 1 4 th grader at Burnt Chimney Elementary School will create a power point from a template for their IEP meeting as measured by the use of the power points at their IEP meetings.

4 Determine (What is our goal?) In Spring of 2007, all 3 rd, 4 th, and 5 th grade students with special needs will create a notebook of personal information to be used to develop their IEPs as measured by the finished product.

5 Determine (What is our goal?) In Spring of 2007, a team of special education teachers will create brochures and posters to be used in presentations with both students parents and teachers as measured by finished products.

6 Evaluate (What do we already know?) Students… Don’t know what an IEP is Don’t know their likes and interests Not familiar with their own disability Poor decision makers Get along with their peers Knows hard work will help them get ahead Believe their teachers listen to them Aware of their needs

7 Think (How do we measure our goal?) Use the IEP Participation Continuum Checklist Use of completed power points, notebooks, brochures and posters.

Oct. 13 - Met as committee for involvement in pilot program/set up goals Oct. 16 - Complete teacher/parent/student surveys for all 5 th graders and SWD at 3 rd and 4 th Oct. 23 - Complete parent survey Nov. 28 - Parent to Parent meeting Dec. 5 - Meet as a committee to review survey data, set up action plan, go over resources Jan. 3 or 4 - Plan sessions and interview End of Jan Beg. Of Feb. - Start working with students to increase self-determination and begin to get them ready for IEP process Feb 13 - Committee meeting March - Student/parent interviews End of March - Begin IEP meeting
