3 The Brief Action Planning Prototype is a Ministry of Health, Patients as Partners initiative administrated by ImpactBC Patients as Partners INTEGRATED PRIMARY ACUTE AND COMMUNITY CARE

6 Why the BAP Prototype In a survey in of Self Management Support skills in BC in: – 87% said patients or clients need more support then they are able to give – 2/3 or above said they are interested in training in goal setting, action plans, problem solving and follow-up. – 64% preferred web based training Patients as Partners INTEGRATED PRIMARY ACUTE AND COMMUNITY CARE

7 Aim of the prototype We aim to provide a method for health care professionals to learn Brief Action Planning (BAP) through a self-paced web training module, coaching and certification program, from June 2012 – October 2012. Such That: – 90% Participating Professionals rate their confidence in using BAP as 7 or higher out of 10 – 70% of all professionals who complete BAP training are certified by March 31 st 2013 – Each person trained in BAP skills applies them to 10 or more patients interactions by March 31 st 2013

Confident BAP practitioners Online Training And Certification Basic skills, certification of BAP proficiency 1:1 Coaching for Learning and Feedback Specific feedback, questions and answers, BAP support Group learning and support (teleconferences) New concepts, problem solving, Q&A, team sharing Measurement and feedback on group progress Uptake of training and coaching, confidence, Use of BAP, Patient Impact
