3. Proposed Procedure in Data Capturing
Current and new members must fill-up and complete PSME Membership Application Form.
Submit signature specimen signed within a space of 3” x 6” using black fine-tipped pen (1/16” wide line) .
Picture capturing maybe taken:
During GMM picture-capturing;
Submitting digital image ed to
Submission of 2 x 2 or passport size colored photo.
Committee on Membership shall collect the data to a database.
ID card shall be issued to members based on the data submitted.
ID card and printing cost P75.00.
