11 A complete application file must include: B.N.A. program application Proof of payment of application fee ($50) and non-refundable fee for program acceptance ($100) The following will already be “on-record” for you if you completed them as defined in the admission packet: Attendance at advising session, College Application, proof of health requirements completed, proof of meeting reading and writing admission requirement.

12  Tuition  Fees  Textbooks Tuition Use the Class Schedule for semester enrolling in. Tuition is noted in the Table of Contents. Fees are identified by the course in the actual course schedule. Textbook costs can be confirmed with the college bookstore. You also can purchase these from online vendors.

14 FINGERPRINTING COMPETENCY EXAM  During the first week of the course you will have FINGERPRINTING done by Accurate Biometrics.  You must bring a CASHIER’S CHECK OR MONEY ORDER made out to Accurate Biometrics in the amount of $40.00 for that service.  Nearing the end of the course you will have to apply for your state competency exam. This exam is coordinated by S.I.U.C.  On the course calendar your instructor will identify a due date for this payment. On that date, you must bring in a CASHIER’S CHECK OR MONEY ORDER made out to S.I.U.C. in the amount of $65.00 for the exam. Note that this passing this exam is required to get on the health care worker registry.

15 1) Textbooks must be purchased before the first class session. Textbooks will be available one week prior to the start of the course. Required:  Nursing Assistant: A Nursing Process Approach, by Hegner, Acello and Caldwell ISBN #1428312528, 10th Edition. To order directly, call (800) 347-7707.  Fundamentals of BLS for Healthcare Providers, ISBN #0-8749- 3461-3 (CPR book is non-refundable as a new book.)  Certified Nursing Assistant Syllabus – available in C.O.D. Bookstore or print of web page. Needed for first class session. THIS MUST BE A NEW, UNUSED COPY! Optional:  Nursing Assistant Workbook, ISBN#1418066095

16 2) Blood pressure equipment (stethoscope and cuff), clinical dress and other noted supplies/equipment must be purchased by the student. Your instructor will discuss the purchase of these supplies during the first class session. 3) COD student ID must be purchased before the first class session. IDs are available in the Student Services Center (SCC) 1217. The cost is $5, and it is good for 3 years.
