2 Annual Open Enrollment  The Glendale open enrollment period for the 2011/2012 employee benefit plan year will begin May 2, 2011 and conclude on May 31,2011.  Open enrollment changes will be effective July 1, 2011.  This is your once a year opportunity to add, drop or change existing coverage.  Next opportunity to make a change without a qualifying event will be May 2012 for a July 1, 2012 effective date.

United Specialty Vision Vision benefit has been unbundled Dependents can be enrolled without enrolling in the medical insurance. Examination ($15 co-pay, every 12 months): Examination ($15 co-pay, every 12 months): Receive a comprehensive eye examination from a state licensed optometrist or ophthalmologist, covered in full, after exam co-pay. Materials ($30 co-pay, every 12 months): The materials co-pay is a single payment that applies to the entire purchase of eyeglasses (lenses and frames*), or contacts in lieu of eyeglasses.

Minnesota Life Optional Life Employee:  An amount between $10,000 and $500,000, in increments of $10,000, not to exceed 5x basic annual earnings.  Benefits cease at retirement. Spouse:  Spouse Optional Life coverage may not exceed 50% of the employee’s coverage.  An amount between $5,000 and $250,000, in increments of $5,000. Children:  14 days – 6 months = $500  6 months – 19 years An amount between $2000 and $10,000, in increments of $2,000.

Short Term Disability (STD) Voluntary STD offered by Union Security Insurance  Benefit up to 66 2/3% of your monthly salary  Elimination period: 0 days injury, 3 days sickness  3 Month Duration  Online enrollment AND paper enrollment form is required. PRE ‐ EXISTING CONDITIONS – No benefits are payable for disabilities that commence within 12 months of your effective date if you received medical treatment, consultation, care or services, including diagnostic measures, or took prescribed drugs or medicines for the disabling condition in the 12 months just prior to your effective date. District Paid STD offered by UNUM  Benefit up to 66 2/3% of your weekly salary up to $1,155 per week  Elimination period – 90 days  3 Month Duration  Automatically enrolled

WageWorks Flexible Spending Account –  Contribute to up to $3,000 pre-taxed per year to medical spending account, $500 pre-taxed per year to limited spending account, and $5,000 pre-taxed per year to the dependent care savings account.  Use it or Lose it rule  Should you enroll in the new HSA $2,500 plan, you can enroll in a Limited spending account to receive reimbursement for Dental and Vision expenses.
