Fellows’ Activity Log The ASRS Fellows’ Activity Log was developed to standardize training of ASRS fellow-in-training members ASRS contracts with Resilience Software (T-Res) to maintain a HIPAA compliant fellows activity log database Designed to let fellows log their activities and directors monitor the progress Fellows and Fellowship Programs have access to individual trainee and program data. ASRS Membership and Activity Log Registration
Activity Log is currently only open to ASRS members All FIT members are required to actively participate in the log in order to maintain an active ASRS membership Upon completion of fellowship, all FIT members are required to submit a completed log in order to be eligible to upgrade to a regular member All fellows are required to export and submit the Report 094 – ASRS Goals Report within 6 months of completing fellowship Individuals applying for regular membership who did not submit a complete Activity Log during fellowship will also be required to complete and submit a log in order to become a member

The Fellows' Activity Log mobile device interface allows users to easily track all activities via their iPhone, Android, or other mobile device. (Below video can be viewed in slideshow mode or at asrs.org/fellowsactivitylog)

Fellows’ Activity Log Fellowship Director Accounts Fellowship Directors also have the ability to have an account to monitor their fellows’ activities. Access allows directors to manage dropdown menus (e.g. attending physicians list, site of service list, etc.) as well as pull reports to track your fellows’ progress. Fellowship Directors interested in having an account created for the Fellows’ Activity Log can to be set up.

Forms to Complete Procedures: This form is used to record individual patient encounters. De-identified information from the procedures forms, such as diagnosis and procedures, is included in the benchmarking database to allow for aggregate data reporting. Clinical: This form enables fellows to record daily the number of clinical patient evaluations, diagnostic studies ordered, and clinical procedures performed. Academic: As part of fellowship training fellows are responsible to serve as medical educators. This form allows fellows to record and track number of hour in didactic instruction, including conferences, lectures, courses, grand rounds, etc. Research: Fellows are required to be involved in research activities. This form will enable fellows to record research activities in a format that is easy to export into a CV.

Now Available: Fellows’ Activity Log Export for AUPO Exit Survey
An export from the Fellows' Activity Log is now available that can be imported into the AUPO Fellowship Compliance Committee (AUPO FCC) exit survey system. All FIT members will now be able to take advantage of this new feature that allows you to log your activities all in one place. Visit the ASRS website for a step-by-step guide on the process:

Tips to Save Time Logging
Logging in all 4 sections of the Log is a requirement of membership - be sure to continuously log in all sections throughout your fellowship so you don’t have to spend time updating it at the end Download the T Res 2 Mobile app on your iPhone or Resilience T-Res app Android to log while on the go. Fellows can make a copy of their last activity logged and update details as needed to reduce duplicate data entry Under Academic forms make sure to select your role (Learner or Presenter/Teacher) to ensure that your entry saves correctly.
