Goals of an FQHC QI Plan Reflects our Mission and Goals Assures & improves the quality of our clinical and management services Communicates improvements to staff, board and patients Complies with FQHC program requirement

Board Responsibilities Review annually Ensure Board Committee oversees QI activities Assure qualified clinical staff, (Clinical Director) has responsibility QI, and is adequately supported Complies with FQHC program requirement Assure record keeping and reporting systems adequately provide information enablineg QI, while ensuring patient confidentiality.

Questions Board Members Should Ask: What is quality? safe, effective, patient-centered, timely efficient and equitable. What are the QI activities and goals? Who is leading the QI Program? Are QI staff trained and competent? Is performance being improved? Is the EMR system adequate to support the QI function?

Six Sigma DMAIC model for Quality Improvement Define – selecting high-impact projects & understanding which metrics will impact success Measure – evaluate & document the process Analyze – collect and analyze data which helps to identify trends & and provide feedback Improve – consider, evaluate & select solutions Control – scheduled revisit of metrics to ensure improvements are sustained
