care coordination remains a function and responsibility of the whole health system

7. Cancer nurse coordinator - specifications
DHBs need to:
fund new clinical positions
focus on the front end of the pathway, not oncology services
support streamlining services and referrals between DHBs
collaborate regionally when developing and implementing these roles
Objective is to:
improve the experience for patients, including their family and whānau, with cancer or suspected cancer
improve overall access and timeliness of access to diagnostic and treatment services for patients with cancer

the problem that the coordinator roles are solving needs to be clearly defined before implementing the roles
coordinators must have the appropriate training and skills
the coordinator roles require the mandate to make changes to systems and processes if they are to improve how patients’ journey through the system without this mandate the coordinator roles will just be filling the gaps in systems and processes
need to educate other health professionals about cancer so all health professionals remain responsible for coordinating care, this means that the coordinators can focus on high and complex needs patients
