Direct booking from NHS 111/IUC Into GP Extended Hours Hubs Healthy London Partnership Digital Transformation Team Guidance described within ‘Refreshing NHS Plans 18/19’ aligns greater focus towards the impact on patient flow when collective booking across services are in place. London has been leading in the national effort to roll out direct booking through NHS 111/IUC services which is already at 98%. London’s UEC commissioners collectively agreed to prioritise booking capability into same day urgent care. HLP started by examining the flow of bookings across the system, with attention towards the breakdown of the service type, and began to map patient flow via IUC/CAS. Having the booking functionality rolled out to GP extended hours hubs already has been critical to the success of IUC CAS roll out.. What we did Scope of impact Over 40,000 appointments booked via GP extended hours hubs for patients since February 2018 Bromley Bexley Greenwich Lewisham Southwark Lambeth Croydon Sutton Kingston Merton Wandsworth Richmond Havering Redbridge Barking & Dagenham Newham TH Hackney Waltham Forrest Enfield Barnet Haringey Camden Islington Central London City WL H&F Brent Harrow Hillingdon Ealing Hounslow HLP supported continued integration of primary and urgent care services, enabling NHS 111/IUC services to book patients directly to GP extended access hubs. HLP supported London’s 5 STP’s to prioritise NHS 111/IUC booking into GP Extended hours hubs in 18/19 and London now stands at 98% supported by this booking function. London is the first region to achieve this This alone makes London Region compliant against the National IUC booking target of 30% target for booking patients in f2f appointments currently reporting green at 32%. Total bookings made into extended hours services in 18/19 amounted to bookings for face to face appointments 43% increase in bookings between Q3 & Q4 2018 98% rolled out to 98/100 GP Extended hours hubs Additional 2 hubs in Croydon now being prioritized Benefits Next Steps 1. Remaining 2 hubs to go live in June STP’s are now now a focus for roll out to ensure London is offering 100% of the population access to a direct booking System-wide benefits Unnecessary walk-ins in to emergency departments will be reduced – Evaluations have shown 4 in 10 Londoners tried to get an appointment with a GP before presenting them selves or a dependent to A&E 1 Improved quality of NHS111 referral/bookings by having direct bookings into GP Access hubs can feedback to NHS111 to improve pathways / CAS triage Feedback has helped support national development of the GP Connect standard based approach. Benefits to patients and Staff More convenient for patients to be seen by a clinician at the most suitable site, following an initial NHS 111 assessment by a health adviser and/or clinician for an urgent primary care appointment. Seamless patient journey for unscheduled care needs meets the growing expectations of more up to date digital offers to patients 2. Ensuring current slots are utilised and progressing Direct booking from NHS 111/IUC services into Primary Care Extended Access Hubs . STP’s are now reporting on utilisation of available slots locally however this information remains limited. HLP are now working to help STP’s understand their capacity/demand requirements.
