Top right: Preoperative arteriogram shows a filling defect in dome of aorta with absence of left common carotid and left subclavian arteries. Findings at operation (top left) consisted in detachment of intima from ascending aorta with intussusception of intimal tube into distal aortic arch producing obstruction of left common carotid and left subclavian arteries. Bottom right: Post-operative arteriogram shows good appearance of graft with normal filling of great vessels. There is still evidence of unresected dissection of innominate artery.
