3 Fig 2 Surgical technique. (A) A double-armed extended polytetrafluoroethylene (ePTFE) suture without pledgets is passed through the tip of the posterolateral papillary muscle. (B) The suture is brought up and passed through the atrial side of the prolapsed posterior mitral leaflet (PML) segment a few millimeters away from the free edge of the leaflet. (C) The length of suture is determined after testing the competence of the valve by filling the left ventricle with saline solution. Once the length of the ePTFE chorda is adjusted, a clamp is placed on the suture to avoid sliding of the knot while tying the sutures. (AML = anterior mitral leaflet.)
