2 Fig 1 The Chord-X Pre-Measured Loops mitral valve repair system. A set of 3 premeasured chordal loops are configured with a single pledget. The pledget is, in turn, affixed to 2 sutures with needles; these needles will be passed through the papillary muscle head and the sutures tied over a second pledget for secure attachment to the papillary muscle. Each loop is also attached to a pair of sutures, each with a needle; these sutures are used to attach the chordal loops to the leaflet edge at the site of prolapse. The dashed circle indicates the components that are retained in the patient. 3 Fig 2 Sizing caliper for determining chordal length. The sizing caliper is inserted into the ventricle with the tip positioned at the papillary muscle insertion of an adjacent normal chord. With the leaflet held under tension, the caliper is adjusted to determine the length of the normal chord. The measured length and the corresponding color displayed on the handle of the caliper indicate the appropriate chordal loop length. 4 Fig 3 Technique for insertion of chordal loops. (A) After sizing, the 2 papillary muscle sutures are passed through the tip of the papillary muscle and a second pledget is added. (B) The papillary muscle suture is tied, affixing the chordal loops to the papillary muscle. (C) Each chordal loop is affixed to the free edge of the leaflet by passing each of the 2 attached needles through the leaflet; for each loop, the sutures should be separated by no more than 5 mm at the leaflet edge. (D) The sutures associated with each loop are tied, thereby securely attaching the chordal loops to the leaflet edge.
