1 Housekeeping: CME Information Accreditation & Designation Statements
This activity has been planned and implemented in accordance with the accreditation requirements of the Washington State Medical Association through the joint providership of Providence Health System and Alaska State Hospital and Nursing Home Association. Providence Health System is accredited by the WSMA to provide continuing medical education for physicians. Providence Health System designates this live activity for a maximum of 5.5 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

2 MOC2 Statement for ABP, American Board of Pediatrics
Successful completion of this CME activity, which includes participation in the activity, with individual assessments of the participant and feedback to the participant, enables the participant to earn 5.5 MOC points in the American Board of Pediatrics’ (ABP) Maintenance of Certification (MOC) program. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABP MOC credit. 

3 MOC2 Statement for ABIM, American Board of Internal Medicine
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 5.5 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

4 MOC2 Statement for ABA, American Board of Anesthesiology
This activity contributes to the patient safety CME requirement for Part II: Lifelong Learning and Self Assessment of the American Board of Anesthesiology’s (ABA) redesigned Maintenance of Certification in Anesthesiology Program® (MOCA®), known as MOCA 2.0®.
