New journeyman had just toped out as a journeyman Age; approximately 26

5 Job Re-conductoring existing 3 phase line.
Old phases were hot and laid out on hot arms. (primary voltage unknown) No cover-up being used.

6 Job Crew tying in new conductor; Crew #1 went one direction on job,
#2 - new journeyman went different direction by him self to tie in new conductor with a bucket truck.

7 Job Crew # 1 finished their job and went to lineman #2’s location.
Crew #1 found bucket in the air and no lineman around. Crew #1 lowered the bucket and found lineman #2 in the bucket dead from electrocution.

Supervisor in the lead driving a pickup Crew is trying to follow supervisor in pickup

11 Supervisor came to an intersection
Supervisor had a private vehicle coming from his right. (clear unmarked intersection) Supervisor hit the private vehicle in the driver’s door.

12 Accident Driver of private vehicle was not wearing a seat belt and was thrown through the wind-shield. Supervisor was pined in his vehicle. Supervisor was paralyzed from the neck down for a while but has recovered. Supervisor has two broken arms and is recovering.

13 Accident Supervisor and public drivers were not wearing seat belts.
It was estimated by law enforcement that the supervisor impacted the public driver at approximately 85 mph.

16 Job Change out 34.5, 3 phase, UG distribution transformer because of oil leaking from bushing.

17 Job Crews; (2 – 2man crews) Equipment; 1 service truck, 1 digger truck
Weather; Overcast, light rain

18 Job - Incident #1 On way to transformer change out; Service truck was in the process of making a left hand turn from highway to side road. Service truck had turn signal on. Private vehicle was passing service pickup during left hand turn. Private vehicle hit service pickup in left front fender (no one injured)

19 Incident #1 Service pickup had to be haled from accident sight.
Original 4 man transformer crew is now split up because of the vehicle accident. 3ed. 2 man crew now joins digger truck crew. $5, damage to service pickup (driver of private vehicle tried to pay off line crew)

20 Incident #2 2 man crew went to dip pole & opened 34.5 distribution UG cut-outs. Crew pulled primary elbows & covered them with a plastic bag to protect them from the rain. Crew pulled secondary.

21 Installing New Transformer
Lineman were guiding primary wire up through bottom of new transformer. Lineman #1 had 2 very small burn marks on hand Lineman #2 had 1 very small burn mark on hand.

Was it capacitance discharge from the ungrounded cable ??? Did the break-up of the original crew contribute ???
