Introducing the Diabetes Team
We both cover different surgeries. Liz : Filey, Humanby, West Ayton, Snainton Falsgrave, Belgrave, Castle Health, South Cliff Karen: Malton, Sherburn, Rillington, Ampleforth, Prospect Road, Peasholm, Danes Dyke, Hackness Road, Brooksquare We work Geographically but cover all surgeries in the absent of each other Caring, Learning and Growing

Main Responsibities Management of Diabetes in Type 1 & Type 2 Patients. Starting new treatments, triple therapies, Insulin and injectable therapies. Complex Medical needs, Co–Morbidities. Structured Education for Type 1 & Type 2 Diabetes Support & Education for all Community staff, Primary Care Staff & Relatives/Carers Caring, Learning and Growing

However we are not an Acute Service our working hours are: Monday – Friday 08.30am – 16.30pm and we are contactable via CAS: Caring, Learning and Growing

Structured Education for Type 2 Diabetes
Good 2 Go Referral Form attached to CAS No paper referrals accepted any longer Caring, Learning and Growing

Structured education for Type 1 Diabetes
DAFNE Course Referrals to CAS This course is predominantly for Type 1 to allow them to be self directed in managing their Diabetes. Caring, Learning and Growing
