2. Introducing the Diabetes Team
We both cover different surgeries. Liz : Filey, Humanby, West Ayton, Snainton Falsgrave, Belgrave, Castle Health, South Cliff Karen: Malton, Sherburn, Rillington, Ampleforth, Prospect Road, Peasholm, Danes Dyke, Hackness Road, Brooksquare We work Geographically but cover all surgeries in the absent of each other

Management of Diabetes in Type 1 & Type 2 Patients.
Starting new treatments, triple therapies, Insulin and injectable therapies.
Complex Medical needs, Co–Morbidities.
Structured Education for Type 1 & Type 2 Diabetes
Support & Education for all Community staff, Primary Care Staff & Relatives/Carers

This course is predominantly for Type 1 to allow them to be self directed in managing their Diabetes.
