4 Safety Culture Climate Survey
To complete a practice Safety Culture Climate Survey – ‘Safequest Submit an action plan by 30th March QOF Reinforce that the safety climate survey is a part of this years QOF, and the requirements of practices

Care Bundles Decide who will be involved Regular data collection – 10 pts every month Populate QIDs / run charts Share your data with team Improvement -Tests of change - PDSAs Reinforce frequency, sample size and data collection process Encourage practice to do PDSAs – they don’t need to be completed formally, as long as they’re written down somewhere!

Explain the innovation adoption curve – the testing work was undertaken by the early adopters, now the challenge is to spread to the majority 7

8 Rapid Organic Spread 60 Practices + 6 HB
Spread to 90% practices in 2 HB 3 more boards testing GP training GP Appraisal There has already been rapid spread of the programme – initially there were 60 practices in 6 boards, this has since spread to include 90% of practices in 2 boards, with an additional 3 healthboards participating. The programme tools are now part of GP training, and can be used as evidence for GP appraisal. Both the trigger tool and safety climate survey have been included in this years QOF
