E – Efficient Care: Patient care will avoid waste of time, money, and resources. E – Equitable Care: Access to care will be provided to all in an equitable manner. P – Patient-Centered Care: Patients will participate fully in care decisions.

Number of falls Number of falls risk assessments completed and who completed by Where falls occur and in what population group? How are we completing our risk assessments? Has this changed over time? Are there other factors we need to be aware of? Measuring performance Measuring safety Improving safety What happened? How did this happen? Triangulating information to support learning

Think about your role in your setting on a day-to-day basis Consider how often and in what format you use data and information related to safety Highlight the section which best matches YOUR experience Collect individually on paper and then get whole group to assess on the whiteboard. In second workshop give time to complete individual assessment and review whiteboard, but not in depth.
