1 Longitudinal Characterization of Cortical Lesion Development and Evolution in Multiple Sclerosis with 7.0-T MRI In patients with multiple sclerosis, 7.0-T MRI detects cortical lesions (higher for intracortical than leukocortical lesion type [1.3 ± 1.7 vs 0.7 ± 1.9, respectively; P = .04]) more frequently compared with previous studies at lower-field MRI strength, and shows no relationship with the rate of white matter lesion accumulation. Cortical lesions preferentially develop in cortical sulci, indicating a possible link with an ongoing cerebrospinal fluid–mediated neuroinflammatory process. Assessment of cortical lesions should represent a main component in the evaluation of progression of disease burden in multiple sclerosis. Axial 7.0-T T2*-weighted images in a 59-year-old with secondary progressive MS shows leukocortical lesions (white arrows) and intracortical lesions (black arrows) along with juxtacortical and periventricular white matter lesions.
