What does that cover? Assisting client’s with transport needs that arise as a direct result of a their injury. Identifying transport solutions to assist client’s to return to employment, access the community and engage in pre injury activities Options considered: Public transport Taxi’s/funded transport Rental vehicle Modifications to existing vehicle Vehicle purchase Vehicle purchase and modification Copyright (c) ACC

4 Determine Transport Needs Form
ACC 4506 was developed to be completed by the case owner with the client. Vital part of there assessment process to gather information and set expectations Expectation that this is fully completed by case owner and accompanies the referral to the assessment team. Copyright (c) ACC

5 Standard Transport for Independence process
7 Vehicle is transported to modifier 3 Assessor completes assessment with client, gets a quote for modifications from vehicle modifier, and submits report to ACC identifying vehicle makes, models, features, & modifications needed. 8 Modifier completes modification and gets certification Record 1 ACC completes Determine Transport Needs form in consultation with client ACC4506 6 ACC pays its contribution to vehicle supplier. Client pays their contribution to vehicle supplier 4 ACC confirms its contribution toward vehicle purchase and/or modifications and issues a decision letter ACCVM01 Vehicle handover to client. Assessor confirms modifications completed & arranges driver training if needed 9 ACC6268 Manual 2 ACC makes referral to Transport For Independence Assessment Service (TIAS) ACC4507 Client sources suitable vehicle. 6 Highly Specialised Transport process
7 Client & assessor visit Specialised Vehicle Modifier. Specialised Vehicle Modifier completes stage 2 modifications 3 Assessor completes assessment with client and submits report to ACC identifying vehicle makes, models, features, & modifications needed. Specialised Vehicle Modifier provides quote for modifications ACC4508 Quote 8 Modifier completes modification and gets certification Record Specialised Vehicle Modifier sources vehicle 5 6 Specialised Vehicle Modifier completes Stage 1 modifications 4 ACC confirms its contribution toward vehicle purchase and modifications and issues a decision letter ACCVM02 2 ACC makes referral to Highly Specialised Transport Assessment Service (HSTAS) ACC4506 10 Vehicle transported to local dealer 9 Assessor confirms modifications completed & arranges driver training if needed ACC6268 Vehicle handover to client Manual 11 1 ACC completes Determine Transport Needs form in consultation with client Copyright (c) ACC

10 Vehicle Modifications
ACC went to tender in 2017. Two National Suppliers of the Level three modifications Ten suppliers of the Level one and two modifications Level two: Modifications that will enable the client to travel seated as a passenger in their wheelchair in the rear of the vehicle; -Travelling in rear of Toyota Hiace Level three: Highly specialised modifications requiring structural changes to the vehicle Sourcing and modification of vehicles -Self drive Mercedes Sprinter Level one: Standard modifications including -hand controls -left foot accelerator -Wheelchair loaders -ancillary controls Copyright (c) ACC
