Epicardial Arrhythmogenic Substrate Attenuation by General Anesthesia Top and bottom panels show the comparison between BrS epicardial substrate with (bottom) and without (top) the presence of type 1 ECG pattern. The top panel shows—in sequence from left to right—the spontaneous type 1 ECG pattern while awake and its attenuation after GA. In the absence of typical BrS pattern, the potential duration map (PDM) (middle position) demonstrates a small area (2.9 cm2) of abnormal EGMs ≥180 ms (purple area in CARTO map). Examples of prolonged- and normal-duration EGMs collected, using a decapolar catheter, are shown beside the map (top panel on the right side, from top to bottom). The bottom panel shows the ECG (on the left) and the PDM (middle position) after ajmaline challenge. The occurrence of type 1 ECG pattern results in enlargement of the AS area (31.5 cm2 area). Examples of abnormally fragmented and prolonged EGMs found in the purple area are shown beside the map (bottom panel on the right side, from top to bottom, respectively). Giuseppe Ciconte et al.
