Co-op funeral care in Wallasey ended up mixing up two different bodies and the families buried someone else and not their loved one. The reverend who was doing the service even notice the name and date on the coffin was wrong, but was told it was the right body and a mistake hadn’t been made. Eventually realizing their mistake they exhumed the bodies and swapped them over without the families knowing. Not only did they not follow the correct procedure in checking the correct bodies into the coffins, but to add insult to injury they were dishonest and disrespectful about the whole situation by swapping them on the quiet. Janina Kolkiewicz was declared dead as her niece found she wasn’t breathing, the doctor looked for a pulse and couldn’t find one and pronounced her dead. 11 Hours later technicians went to retrieve her body from the fridge and saw that she was moving. Even though the doctor thought he was following proper procedures, the lady must have still had a very faint heart beat or pulse for her to still be alive. The practising doctor should have taken more care in examining her. Lucky Janina had late stage dementia and didn’t know what had happened to her, she just complained that she was a bit cold. If information is required or assistance from another team member, you would use for communication. Assisting pathologist with an autopsy – handing instruments, weighing organs, taking blood, fluid or stomach samples, taking samples of organs for microscopic slides. A FUNERAL firm has apologised to two families after first burying the wrong man - then exhuming the coffin without permission and secretly replacing it with the correct one. The atrocious blunder by Co-op Funeralcare was made despite a vicar spotting that the coffin being lowered into the grave bore the completely wrong name. When Reverend Andrew Mannings discreetly raised his concerns - bluntly saying he believed there had been “a dreadful mistake” - the female funeral director instead insisted to him that the deceased had two names, and that the correct body had been buried. But moments after the vicar and the bereaved family had left the graveside, the Co-op staff realised they had indeed made a terrible error. The issue was later fixed but the damage had already been done as the 2 separate families were different religions so the service wouldn’t of worked for them. Computers – database. Information for research and to store peoples medical records. Orally - whiteboard and lecturing. Learning on the job and orally recording process of results e.g. autopsy. Posters & signs.- Safety posters for using equipment and chemicals for embalming. Worksheets & Books.- Technician may need to look up symptoms or medical knowledge to identify an issue or cause of death. Safety and emergency information,- Eyewash or fire blanket. Chemical used and dangerous instruments e.g. Bone saws, syringes etc. Especially if the cadaver suffered with HIV or AIDS.
