3 TIMELINES

4 Upcoming deadlines Late March/April 1 release of RFP-more competitive as result of awareness, increasing need May Legislative Final Approval Beginning on May 1, 2015, and May 1 of each year thereafter, subject to available appropriations, School Health Professional Grant applications will be due for funding available July 1 of the subsequent fiscal year.

4/24/2019 Evaluation & Reporting Each Education Provider that receives a grant through the School Health Professional Grant Program are required to report, at a minimum, the following information to the Department on or before March 15, 2015: The number of school health professionals hired using grant moneys; and A list and explanation of the services provided using grant moneys. Any revisions to budget following award These are a non exhaustive list of metrics that support the attainment of the program’s 3 main purposes and goals The number of school health professionals hired using Grant moneys. Number and grade levels of students served. School health professional to student ratio. A breakdown of the use of matching and matched Grant moneys. A list and explanation of the substance abuse, intervention, and behavioral health care student services provided using Grant moneys. A list and explanation of the training and resources for school staff provided using Grant moneys. Any health-related professional development programs provided using Grant moneys. Information indicating an increase in the level of evidence-based programming on substance abuse prevention education provided to secondary students at recipient schools, such as screening or referral care programs. A list and explanation of utilized community-based organizations for treatment and counseling for students. Evaluation of impact of the School Health Professional Grant Program, to include a comparison of the numbers of students served, student outcomes, etc. if applicable, at the recipient secondary schools for which the Education Provider receives a grant. 6

A list and explanation of the substance abuse prevention education and early interventions used in order to promote the behavioral health of students using Grant moneys. Due: June 1, 2015 to Contact program manager with questions

A list and explanation of the training and resources on substance abuse prevention and early intervention for school staff provided using Grant moneys. Any health-related professional development programs provided using Grant moneys. A description of the increase in the level of evidence-based programming on substance abuse prevention education and early intervention provided to secondary students at recipient schools, such as screening or referrals to community behavioral health care programs.

Guidance: All changes to salary or equipment must be submitted to program and fiscal for prior approval-increases or decreases.  Changes on other budget lines that are for 10%  or less than the district’s allocation can be made without prior approval if it is Purchased Services, Other Purchased Services or Supplies.  The budget must be updated to reflect all changes.
