3 Next Steps... Established a ‘Case For Change Group’- Strategic Group
Met with Loch View staff to advise them of future plans Prepare a paper for the NHS Board and both Integration Joint Boards - developed by the ‘Case For Change Group’ Developed a ‘Delayed Discharge Group’- Operational Group

4 Challenges... ‘Case for Change Group’ - monthly
-Anxiety around the viability of the service, job security -Carers anxiety -Time pressure - 9 months ‘Delayed Discharge Group’- monthly -Understanding of roles(why am I here?) -Some felt that their work was being criticised -Some felt that meetings were too often and time constraints were too tight.

5 Managing Challenges- Bed Closure
Clear message to all staff (regular updates, ‘drop in’ sessions) No redundancy policy- Staff Side Reps involved Opportunity to do things differently (in reach/ outreach) Meetings with carers/ some patients (SALT involved)

6 Managing Challenges- Delayed Discharge Group
Clarity around everyones role True team work - Housing, Social Work, AHPs, Nursing, Medical, Managers, Health Improvement Scotland(HIS) Clear and achievable timescales. Person Centred, Safe and Effective Discharges.

7 Benefits & Outcomes Learning each others roles in greater depth
Learning each others processes and pathways - overcoming barriers Processes and Templates were shared with other areas of Scotland - HIS Positive outcome for patients and family/carers Successful reduction of 6 beds Savings achieved

8 Next Steps.. Revisiting the nursing establishment within other wards
Developing in reach/outreach service Assists with transitions (early discharges) Preventing admissions Continue work with the ‘Delayed Discharge Group’
