NH CARE ACT DECISION TREE
Upon entry to hospital Does patient / their legally designated health care decision maker wish to designate a lay caregiver? YES Declines Consent NO Request written consent (as per hospital’s procedures) to release medical information to designated caregiver Document in medical record. No further action needed. Patient/family receives discharge plan and information upon discharge, according to your hospital’s current practices Document designation in medical record: relationship to patient, name, phone number and address (as available) As soon as possible, consult with patient & caregiver regarding caregiver’s capabilities and limitations. Issue a discharge plan that includes: name & contact of caregiver; description of after care tasks; contact information for resources. If patient is going to be discharged to their residence, the caregiver is notified of the upcoming discharge and will be offered the education and training for their after-care. If the patient is transferred to another facility, the caregiver is notified of the upcoming discharge. Education and training of the caregiver is not required as this will be done per needs determined in new facility. YES Provide & document caregiver necessary training/education in a culturally competent manner, that includes live demonstration and ability to ask questions.
