1. SCDSB Reporting Forms Online Aggressive Incident Report (AIR)
Complete the online aggressive incident form (AIR) if you believe you have been the victim of an aggressive act
An aggressive act may involve a student, parent, visitor, or co-worker, you determine if it is an aggressive incident
Board – Departments - Corporate Risk- Health and Safety – Aggressive Incident Reporting
There must be a follow up meeting with the administrator within 3 days to review the incident
During the follow up meeting Page 2 of the AIR form is completed and changes are made to mitigate the changes of the even occurring again.
Form is sent electronically to the Board where it can be reviewed by JHSC
Violent Incident Form
A violent Incident Form is completed when any of the following occur;
Possession of a weapon,
Robbery or extortion,
Physical assault causing bodily harm requiring medical attention,
Sexual assault,
Using a weapon to cause or threaten bodily harm,
Any hate or bias occurrence
Form is completed (pen and paper) and submitted to administration, (keep a copy for your records) police must be contacted (reference number provided)
No follow up meeting required but members are encouraged to follow up with admin
Safe Schools Form
Part I must be completed for student actions which could result in a suspension or expulsion (see back of page)
Student actions do not have to happen at school or on a school event
You must check all the boxes that apply to the situation and submit to your administrator
A Safe Schools Incident Reporting Form – Part II will be returned acknowledging submission of this form
