1 6055/6056 Reporting Tool Overview
The National General Benefits Solutions Self-Funded Program provides tools for employers owning small- to mid-sized businesses to establish a self-funded health benefit plan for their employees. The benefits plan is established by employer and is not an insurance product. Stop-loss insurance for the Self-Funded Program is underwritten by National Health Insurance Company, Integon National Insurance Company, and Integon Indemnity Corporation. 2 Reporting overview Self-funded plan sponsors that are ALEs must report under both sections, but will report using Forms 1094-C and 1095-C

2019 deadline has been extended to: March 4,2019 IRS returns: 2019 deadline for paper filing and for businesses with fewer than 250 individual statements: Feb. 28, 2019 2019 deadline for electronic filing and mandatory for businesses with 250 or more individual statements: April 2, 2019

7 Login instructions Select the personalized link provided to the Group Administrator in the from Account Support Once you select that personalized link you will be prompted to set your password. Once you create your password, select in the and you will be directed to the login page of the Reporting Tool.

10 Some tips before you get started
The workbooks can take approximately 45 minutes - 2 hours to complete depending on the group size. Make sure you have your census available when starting the form The group census can be downloaded from either the Allied or Business Solver portals. Instructions on how to download are located on the home page of the Reporting Tool, under step 2. Follow the instructions provided within the workbook or download the Employer Instructional Guide from the home page, step 3. The content entered in the workbook will be entered into the forms. Do not use commas and/or hyphens in the workbook. Upon completion of the workbook save it on your computer or a USB drive.

12 Completing the required reporting
Once you have completed your workbook and saved to your computer or USB drive, select the “Launch” button to upload the workbook to the portal. Launch

After you select the Launch button in step 4, you will be directed to the workbook upload page Click the Browse button and upload the workbook from your computer or USB drive Confirm your address this address is where the completed employee ACA documents will be sent Select the Submit button Submit

14 What happens next? An will be sent to the address entered on the workbook upload page. You will not be able to change the address after you hit submit If an incorrect address is entered you will not receive the tax documents The will include the zip file with your individual employee ACA tax forms Print the applicable individual 1095-B/1095-C forms and distribute to your employees File 1094-B/1095-B or 1095-C/1094-C with the Internal Revenue Service (IRS), per the instructions on the forms Due date reminder: Individual Statements distribution: Extended to March 4, 2019 IRS Returns: Feb. 28 , 2019, April 2, 2019 if filed electronically
