1 California Youth Football Association Certification Procedures

2 Paperwork Procedure SAFETY FIRST! Paperwork is required from all participants including Players(Football and Cheer have the same requirements minus the MPR forms. Rostering will be the same.), Volunteers and Associations. It is the responsibility of the conference or any event host to verify that all items on the checklist are complete. Paperwork is for your safety as well as the participants. NO person, team, or conference shall be eligible to participate in any AYF/CYFA interleague competitive event/tournament in football or cheer without completing the required paperwork. **Conferences/Association should keep waiver forms for a minimum of 7 years to protect against liability; laws vary by STATE so you should consult local counsel.**

3 All Paperwork Requirements
Getting Ready for Certification Notes: Certification paperwork will be checked in stations per Division. 1) Each color coded Document on the list will have a separate station that will check the documents in entirety. (There will be a total of 8 stations for each division) 2) Every station should have 1 copy of a roster on top of each group of separate documents for that station, and be submitted in a 9 x12 envelope to that station. 3) To simplify this presentation, each required form/forms (on list /station) will be In player/coaches roster order. 4) There will be 1 document for each rostered player. (Please make sure that the documents are filled out with all requested information in entirety). 5) All documents will be checked and approved by the conference representatives (President /Board member from each chapter) checking the documents for completion and accuracy at each station. 6) All stations will check off players who pass certification , and list all players who did not pass ( a detailed reason will be given) to the conference ADs and or Conference Board member for final certification and a Conference stamped Roster. 7) All players that do not meet all certification requirements will be crossed off the roster, and given the opportunity to rectify and submit any required documents or information needed at Make up Certification on August date TBD. ( location will be given at certification). NOTE: All Teams will be given a scheduled time to show up for Certification. All Staff and Board members shall have the following task completed prior to certification day. Background Check – completed CYFA website (SSCI) by July 31st. Coaches Training certification Volunteer Waiver & Release of Liability – Adult Image Release – Adult Red Cross Certified Volunteer Cards – must have one on site at each game. Play Coaches Concussion Affidavit / CYFA 2018 Rule Book Training.

6 Rosters/MPR The next two pages after the rulebook is your Official Rosters. This is a two-page document and should be placed front to back, in one sleeve protector. 2 Roster copies are required, and both those rosters will be certified by the conference. Be sure to type all information completely. Also note…football and cheer uses different types of rosters

8 It’s now time to put in the paperwork for your participants.
All the remaining participants will behind the Players tab. Players are listed in alphabetical order, by the LAST name. Do NOT put players by the order on your MPR sheet.

So far you should have your rule books, roster, and MPR forms in your binder. The next section is for your players, First document starts with the Participant, Tracking and ID Card. *Many people also call this the PLAYER CARD *Note: Players are to be listed in alphabetical order last name first. Please Note: Jersey Numbers are required on all the Players Cards. This form is used by the league to certify that all the documents are present. We will train you on how this card is stamped at our certification clinic. C

12 VERIFICATON OF AGE 1.) MUST SUBMIT AN ORIGINAL BIRTH CERTIFICATE (NO HOSPITAL BIRTH CERTS . WILL BE ACCEPTED) 2) A PASSPORT WILL BE AN ACCEPTABLE FORM OF VERIFICATION. 3) FOR FOSTER CHILDREN WHO NO BIRTH CERT. HAS BEEN GIVEN, A STATEMENT STAMPED BY THE DEPT. OF SOCIAL SVCS. WILL BE ACCEPTED AS VERIFICATION. SCHOOL IDENTIFICATION SCHOOL REPORT CARD IS REQUIRED FOR ALL PLAYERS AND CHEER LEADERS . SCHOOL STAMPED PRINT OUTS WILL BE REQUIRED FOR ALL THOSE WHO DO NOT SUBMIT A REPORT CARD. NOTE THAT DATE OF BIRTH MUST BE PRESENT ON THE DOCUMENT. 3) HOMESCHOOL STAMPED VERIFICATIONS WITH AGE AND GRADE INFO WILL BE ACCEPTED.

17 Let’s recap the PLAYER section:
Players are listed in alphabetical order, by last name . The first sheet protector should a label tab with the players last name and first initial. There are 8 pages for each player (listed in order), and 2 additional pages must be included for special circumstances. (Your organization may ADD Player forms to the team book (ie: report cards) but MAY NOT leave out any listed here. 1. Participation, Tracking and ID Card – Page 1 (Front Side) 2. 19 CY THE NEXT TAB CONTAINS YOUR TEAM INFORMATION. STARTING WITH BACKGROUND AFFIDAVIT AND COACHES TRAINING. Please note that all background checks will be completed through live scan and must be completed NLT July 15th. All volunteers are required to have a background completed through live scan. Any chapter in violation of this can be fined up to $1000 dollars. Coaches courses will shall be placed in this section for all coaches that includes cheer coaches. Documents required here will be sent out by Louis and Lui for football and for Cheer by Bonnie NLT June 15th. Concussion training is a must and will be required by all coaches and their staff.

23 PLAYER ID CARDS PLAYER ID CARDS WILL BE TAKEN IN THE 2ND WEEK OF SEASON BY SCHEDULED APPOINTMENT. CONFERENCE REPRESENTATIVES WILL MAKE AN APPOINTMENT TO COME TO YOUR LOCATION AND TAKE PICTURES OF ALL TEAM PLAYERS AND STAFFS. WE WILL BE SCHEDULING THOSE DATES THE WEEK BEFORE SEASON ON A FIRST COME FIRST SERVED BASIS. ALL PLAYER AND STAFF ID’S WILL BE ISSUED UPON COMPLETE CERTIFICATION OF PLAYERS AND STAFFS.
