Referral Form
Form ‘cleaned up’ in order to improve patient’s initial visit into our clinic (specifically how quickly a patient gets to a provider)
New form has been sent to Forms department and we are awaiting it’s return

Have been working at increasing awareness of PHS clinic
So far in January, we have received 6 new HIV referrals – one of them a self referral and of the remaining five, four of them were from physicians we have not received referrals from previously

New management asking questions about our clinic. Has increased our awareness of the need to access stats quickly
Worked with EMR team to develop a process for getting CD4 and VL electronically. Process completed, however, need more staff do enter the data!
