We will be using the chat box, watching the powerpoint, and having some handouts.

Where do you live and what do you do in practice ? What will be the focus of your risk assessment? Eg someone who is at risk of malnutrition, falls, or who has a swallowing difficulty or a visual impairment. You are free to focus on any service user for whom a risk assessment is appropriate.( with full consent )

6 THE TASK ! Aims - Write a report / essay
- following specific guidelines (page 2 ) in relation to a risk assessment and associated care plan you have drawn up for a service user in your care, needing a risk assessment. - you will illustrate your knowledge and understanding of risk assessment process from your reading of Block 3 and your service user will be the case study to enable this.

7 Definitions ‘Hazard’ means anything that can cause harm, while ‘risk’ is the chance, high or low, that somebody will be harmed by the hazard.

8 Activity 15.3 What does risk mean to you in your professional life ?
Think of something you do in your daily life that is a hazard, and so carries a risk to your safety or the safety of someone you know. Ask the following questions: Choose one example and feedback to the main room What is the hazard? What are the associated risks? How do you adapt your behaviour or approach to manage the hazard and so reduce the risk? How do other people approach the same hazard and what do they do to reduce the risk?

11 Structure and Layout - Present your work as a report or essay with the four of the five sections of the main body detailed in the guidelines page 2. - As required include an introduction and conclusion. Cut off date 23/3/2017- Use headings and maybe subheadings in line with what is required as this will enhance your structure - Ensure where needed to address all parts of the sections. - Try to allocate most of the word count, say 1200 words, equally between the four sections in the main body. Stay within 5% of the required length.

12 Structure and Layout - You then have 300 words for introduction and conclusion. - State the length before the end of TMA references. - Attach the detailed risk assessment or add as this an appendix at the end after the references (final section.) - Send your TMA through the TMA system as a microsoft word 97/2000/XP format. PAGE FORMAT - Head page 1 – with your name, PI, module code and TMA number - Head subsequent pages with your PI - Number your pages

13 INTRODUCTION – ENSURE YOU MAINTAIN CONFIDENTIALITY THROUGHOUT THIS TMA. - ENSURE YOU SUPPORT YOUR WORK BY REFERENCES TO MODULE OR PRACTICE THEORY WHERE YOU CAN THROUGHOUT THE TMA. CONTENT – Provide an overview of the service user, demonstrating your knowledge and understanding of them as an individual in the context of the risk you are assessing. Explain why it is important to identify and address the potential risk to this service user. NOTES/ EXAMPLE OF PARAGRAPH START My service user/patient/client chosen for this TMA is X. In terms of socio economic factors X …..... In terms of relevant conditions X has ….... 15 MAIN BODY SECTION 1 A. Explore the theory and evidence base underpinning the process of risk assessment and its contribution to safe person-centered practice – generally, and the importance of effective record keeping within this. B. Then explain why the service user’s situation puts them at risk. NOTES/EXAMPLES OF PARAGRAPH STARTS Use headings eg Main Body. Section 1 A. Here using block 3 material unit 15/16 in particular and referencing this – describe – the systematic, legal, standardized confidential process of risk assessment and the importance of this to safe practice – the cyclical risk assessment model - SMART target use in evaluation – and record keeping and its importance with this process

17 MAIN BODY Section 2 The completed risk assessment, included as an appendix to your assignment, should clearly demonstrate a systematic approach to risk assessment. This will include interventions and evaluations of these in a SMART targeted way. NOTES Ensure this is confidential and detailed. Use the form provided or your own. Ensure you have enough detail in relation to your interventions to reduce or remove the risk/s so each of these and your method of evaluation for it follows SMART target model objectives – figure Then you should be able to draw relevant information from the form for the final sections.

18 MAIN BODY Section 3 From the completed risk assessment, discuss the specific interventions you would implement in a plan of care to reduce the risk to the service user and, where appropriate, to others and identify the anticipated outcomes of the plan of care. Section 4 Include a sound rationale for each intervention you propose supported by reference to underpinning theory and the evidence available. Interventions and outcomes should address physical, psychological, social and cultural needs. Notes. It maybe that in terms of structure you wish to produce 2 headed sections. In this case for ease and to save words maybe use a letter to identify each intervention and the same letter in the following section when considering rationale

Be specific with the aims of each intervention. Ensure you have considered the interventions in a way addressing holistic needs. Use practice documentation to support your rationale

21 MAIN BODY Section 5 Discuss how you would evaluate the plan of care against the outcomes you identified. To add depth here do this in a way that includes SMART targeted way. Notes. - This is written in the future tense and again draws from the care plan. Eg - I have used SMART objectives to enable both care planning and evaluation processes ( module reference. ) A. The degree of success of the intervention of will be measured specifically in terms of This measurement of success is supported by ( reference.) This evaluation will be done on by and results recorded in Further evaluation will be done ..... Progress/success of this intervention will be discussed with/at where a decision in relation to success will be made and the intervention changed/adjusted/continued depending on evaluative results. There will be recording of this on B. (second intervention evaluation considered as above. )

22 CONCLUSION Conclude your essay/report with a brief summary of the key points that you have covered. Notes. Here you need a summary of the risk assessment process model and important features in relation to the process. Including a summary of the use of SMART targets would be good here too. Use examples to illustrate from your case study if you wish. What is not required is the story of the patient/client/service user.
