STRATEGIC PLAN SO 1: Reduce the number of deaths, illnesses and impact from disease on mother and children through the implementation of CBHD and promoting access to public health services. SO 2: Reduce the number of deaths, illness and impact from communicable diseases by empowering and increasing community capacity to prevent and respond to communicable diseases SO 3: Reduce the burden of non-communicable diseases and other health problems by empowering and increasing community capacity to prevent and live healthy lifestyle SO 4: Reduce the risk of death and illness and impact from health in emergencies and disaster by working in collaboration with relevant key stakeholders SO 5: Strengthen local/national/international partners and key stakeholders to effectively implement the CRC health and Care in the community program
