2 Developmental Checklist
The Developmental Checklist is not the actual report card. The Developmental Checklist must be entirely completed. If the checklist is not entirely completed the narrative report is not accurate. This checklist helps to create the Narrative Report. The checklist can be used to discuss progress with parents.

3 Narrative Report Card The Narrative Report is the Report Card.
The Narrative Report is the only reporting system that goes out to parents. Teachers should meet with parents to go over the Narrative Report. A letter can also be sent home with the Narrative Report card explaining the domains, and how information is gathered. At Back to School Night the Narrative Report Card should be introduced and discussed.

A Topic Sentence Something the student did well Something that you are working on next Something that the family can work on at home with the student

5 Writing an Anecdotal Note
Anecdotal Notes must be written with Work Sampling on a weekly basis. All anecdotal notes should reference what the child did or is able to do. Write factual information. Never write what the student is not able to do. Record anecdotal notes that are meaningful, and can be applied to more than one area Once you’ve observed a child doing something (saying Good Morning to the teacher, and friends) don’t record that behavior again. Look for other ways the child is showing that skill.
