1. SAU OPT STEM Extension Mailing Instructions
This PowerPoint provides simple instructions to mail your OPT STEM Extension packet to USCIS.

2. Introduction
This PowerPoint will explain how to mail your OPT documents to USCIS. Pictures will accompany where examples will be helpful.
The application must be received by USCIS within 30 days of the date that appears on page 1 on the signature line of your I-20.
**Please note: You need to pay close attention to the address that you write on the envelope. The address is different depending on the address that you used on your I765 form.

3. USCIS Package Checklist:
G-1145 Form Attach this from to the front of your application. This will allow you to receive and text notifications about the status of your application.
I-765 Application. Make sure you sign and date it
$410 fee payment by including either:
Form G-1450 if paying by credit card, or
a personal check or money order payable to "U.S. Department of Homeland Security"
Two passport type photos taken within 30 days of filing the application that meet the requirements on the Department of State website. Write your name and I-94 number lightly on the back of each photo.
Copy of your I-94.
Photocopy of the identification page(s) of your passport, including photo and expiration date.
Photocopy of your current visa.
Copy of your OPT Recommendation I-20 (signed)- DO NOT SEND ORIGINAL!
Photocopy of student data pages of all your previous I-20s. DO NOT SEND ORIGINALS!
Photocopy of your previous EAD card (both sides).
Proof of STEM Degree: Final Transcript (Preferred) or Photocopy of your diploma
showing the major and degree level that makes you eligible for the STEM Extension.

4. IF you are applying with a prior degree not listed on your current 12-Month OPT I-20
• Include a copy of your I-20 from the previous school. If the I-20 is not available, request a screenshot of your SEVIS record from the previous school, or a letter from your previous International Office.
• If you have previously had another OPT STEM Extension at another degree level, include a copy of your previous STEM I-20 and EAD.

5. Preparing the Application Package
The application must be received by USCIS within 30 days of the date that appears on page 1 on the signature line of your I-20. Mark the package with “I-765” in big letters in one of the lower corners.
