Are you and all others involved in the business or undertaking acting on a voluntary basis? Level of Risk Likelihood (chances that harm could occur) 5 Almost certain 4 Likely 3 Moderate 2 Unlikely 1 Very rare Risk Score Consequence (potential outcome) The potential outcome would be Catastrophic (Fatal) 10 9 8 7 6 High Risks with a rating of 8, 9 or 10 The potential outcome would be Major (Serious harm) Risks with a rating of 5, 6 or 7 The potential outcome would be Significant (Time off work/unable to continue usual daily routines) Low Risks with a rating of 2, 3 or 4 Minor (Medical treatment) Significant Risks Significant risks are risks with a rating of 4 or more.
