1. Community Mentorship Program

We are all in this together!

2. The students on the Community Mentorship Team work at a variety of job sites to develop both work and life skills needed to be successful adults, while building solid resumes. As the school year progresses, students demonstrate increased stamina and independence while building positive relationships with their mentors and developing an understanding of the importance of team work. Emphasis on assessing students’ career interests and abilities while maintaining excellent attendance continue to be the focus.

3. Tools utilized for planning and monitoring students’ vocational interests include:
IEP (Individual Education Plan) where vocational goals are identified and monitored
CEVEC Rubric-evaluates students performance
CEVEC Career Assessment-results help develop vocational planning.
PINS – Preferences, Interests, Needs, Strengths
Informal Assessments for Transition Planning tailored to each individual student.
Backwards Planning – uses the IEP, rubric, Career Assessments, PINS, Informal Assessments to help the team develop a plan to help the student achieve his/her goals.

4. As members of this program, our students will:
DEVELOP A GREATER SENSE OF TEAMWORK

5. Participate in the orientation process and consistently follow assigned job duties

6. Model expected work behaviors and perform with professionalism on the job ….

7. Expand their knowledge of company policies including dress code and attendance

8. Demonstrate and apply independence
while on the job.

9. Learn the value of soft skills, executive functions and employability skills such as…

10. and practice employability skills such as time management, grooming, following directions, and monitoring their work quality.

11. Students on the Community Mentorship Team explore vocational interests by participating in actual jobs based on their interests while they build their resumes.

12. Our job on this team is to help students develop skills that enable them to demonstrate an increased understanding of what it takes to make a seamless transition from High School to work.

14. Individual training sites and job placements can be developed on an
individual basis with on TEAM INPUT.

15. Resumes
While on the team, each student will work with a coach to develop a resume that is reflective of their job training and can be brought with them to future interviews. An electronic copy will also be shared with student.

18. Upon completion of the program, students will achieve the ability to:
