2 Introduction Pharmacy practice is the discipline of pharmacy which involves developing the professional roles of pharmacists.

3 Type Areas of pharmacy practice include: 1 Disease-state management.
2 Clinical interventions (refusal to dispense a drug, recommendation to change &/or add a drug to a patient’s pharmacotherapy, dosage adjustments). 3 Pharmaceutical care. 4 Communication skills.

4 Type 5 Patient care. 6 Drug abuse prevention. 7 Prevention of drug-drug interactions or drug-food interactions. 8 Prevention or minimization of adverse events. 9 Drug discovery and evaluation.

5 Community pharmacist 1 Is responsible for controlling, dispensing and distributing medicine. 2 Ensure the correct and safe supply of medical products to the general public. 3 They maintain & improve people’s health by providing advice & information as well as supplying prescription medicines.

6 Community pharmacist 4 They also prescribe or give OTC medical products & instruct patients on the use of medicines. 5 Some pharmacists will offer specialist health checks, such as BP monitoring, diabetics screening, run stop smoking clinics and weight reduction programmer. 6 They work in high street pharmacies, supermarkets and local health care center.

7 Typical works Typical works of community pharmacists:
1 Dispensing prescription medicines to the public. 2 Ensuring that different treatments are compatible. 3 Checking dosage and ensuring that medicines are correctly and safely supplied and labeled. 4 Supervising the preparation of any medicines.

8 Typical works 5 Liaising (to communicate or contact) with doctors about prescriptions. 6 Giving OTC medicines. 7 Counseling and advising the public on the treatment. 8 Advising patients of side effects of medicines or potential interactions with other medicines. 9 Preparing cassette boxes, usually for the elderly and those with memory difficulties, where tablets are placed in compartment for specified day of the week.

9 Typical works 10 Undertaking medicine use reviews, an advanced service to help patients understand how their medicines work and why they have to take them.

12 Pharmaceutical care Old definition (1989)
is the responsible provision of drug therapy for the purpose of achieving definite outcomes that improve a patient’s quality of life.

13 Pharmaceutical care 1997 A practice in which the practitioner takes responsibility for patient’s drug related needs and is held accountable for this commitment, this occurs with the patient’s cooperation and in coordination with the patient’s other healthcare providers.

14 Pharmaceutical care Aims of pharmaceutical care
It is designed to complement existing patient care practices to make drug therapy more effective and safe.

15 Pharmaceutical care Where it can be applied ?
It can be applied in all setting, long term care and the clinic. It can be used to care for all types of patients with any types of diseases taking any type of drug therapy.

16 Pharmaceutical care Who is pharmaceutical care practitioner?
The pharmaceutical care practitioner is a new patient care provider within the health care system, this practitioner is not intended to replace the physician, the dispensing pharmacist or any other health care practitioner.

17 Pharmaceutical care Who is the best to be fitted as this practitioner?
The pharmacist, that is equipped with a minimum of 6 years of academic preparation that focuses on pharmacology, pharmacotherapy and pharmaceutical care practice.

18 Pharmaceutical care What is the job of Ph.care practitioner?
The Ph.care practitioner identifies resolves and prevents drug therapy problems.

19 Pharmaceutical care How Ph.care practitioner can manage all of the patient’s pharmacotherapy? This practitioner‘s responsibilities is to know all products being used to treat medical conditions, to prevent illnesses and to improve the quality of patient’s life. i.e. any medication used by the patient’s whether OTC or POM.

20 Pharmaceutical care The need for this practitioner results from
1 Multiple practitioner writing prescription for a single patient, often without coordination and communication. 2 The large number of medications and overwhelming amount of drug information presently available to patients. 3 Patient playing a more active role in the selection and use of medication. 4 Increase the complexity of drug therapy. 5 Increase in self care through alternative and complementary medicine. 6 High level of drug related morbidity and mortality which results in significant human and financial costs.

21 Pharmaceutical care requirements for Ph.care practitioner:
1 Understand your responsibilities. 2 Develop a therapeutic relationship with each patient. 3 Apply the pharmacotherapy work up to make rational drug therapy decisions. 4 Learn patient care process.

22 Pharmaceutical care 5 Acquire an appropriate pharmacotherapeutic knowledge base including up to date knowledge of drug or drug therapy and good drug information skills. 6 Develop clinical skill. 7 Understand practice standards and ethical considerations. 8 Document all care provided. 9 Communication skills.

23 Pharmaceutical care So pharmaceutical care;
Is a series of interactions between patient and pharmaceutical care practitioner. The patient care process is where the practitioner’s unique knowledge and clinical skills are applied to solve health care problems for patients.

24 Assignment What is the difference between drug withdrawal and drug recall? How the pharmacies know about it? Give an example of each one?
