The right pleural space is opened, the right pleuropericardial fat pad is excised, the right hemisternum is elevated, and deep left pericardial stay sutures are in place. Right lateral decubitus positioning allows gravity to assist in cardiac apex displacement into the right chest with minimal hemodynamic compromise. Suction-based stabilization avoids left ventricular deformation.
