2 What is our vision for Older Adults in Southend?
“It is our aim that the older population of Southend-on-Sea should lead fulfilling lives and be given every opportunity to age well in a community that values their experience of life, whilst also helping them to stay healthy enough to remain independent for as long as possible. This includes the most vulnerable and those with complex needs”

3 Local Challenges in Southend-on-Sea
A higher than average number of people over 65 (18.9% of the population against a national average of 17.6%); A higher percentage of older people living in deprivation than the national or comparable local authority average; Life expectancy is around nine years lower in the most deprived areas when compared with the least deprived areas; Southend has a significantly greater number of older people who live alone than either England or comparable local authorities; Over 20% more older people are supported by Southend-on-Sea Borough Council than the national average

4 Key Older People Strategies
Personalisation: supporting older people to make more decisions over managing their own care, while providing the necessary information and guidance to support this. Prevention: the emphasis has increasingly concentrated on supporting the population to remain healthier for longer. Where support is required, then the preferred option is to deliver it within the community wherever possible and reduce the pressure on primary and acute services. Integration: Partnership working is important to the NHS and Southend Borough Council in order to improve the well-being of all residents in Southend-on-Sea and avoid duplication and inefficiencies

5 Transformation of Social Care
The adult social care transformation programme is a ‘whole system transformational approach’ to change and includes community groups, health and social care.  We are looking at the positives through  strengths-based approach, focusing on individual abilities and community assets, rather than a negative approach that focuses on deficits and services to meet need.  As part of the Locality Approach our social workers are working alongside their health colleagues and third sector partners, to have a strong understanding of the local community.

We are reviewing and simplifying processes Our approach is empowering and a shift away from a narrow focus on providing a commissioned service or product for someone We are working alongside people to help them figure out how best to achieve overall wellbeing and the outcomes that they want to see in their lives for themselves and their families.

7 The Locality Approach The vision for the locality approach is that a locality is the central place where integrated health and social care interventions are delivered and co-ordinated. Each locality is utilising existing (or new) NHS / Council estate to provide primary, community and social care services

We are working in a multi-disciplinary team environment so that people; receive co-ordinated care and, those that need it, have an integrated care plan; have a named professional for all health and care needs; are empowered to engage with community assets receive seamless support.

10 Complex Care Service The vision of the complex care service is to provide complex care people with a proactive service that co-ordinates their health and social care provision based upon existing services and need. The aim of the service is to ensure care needs are assessed, care plans are co-designed through an established multidisciplinary team approach and care is delivered in a co-ordinated way Team is aligned to locality approach

11 An Asset-based Approach
Everyone can contribute and make a difference in Southend-on-Sea The community knows its strengths and assets best, whether these are people, buildings, businesses or relationships; We recognise the need to work alongside the community to develop an approach that utilises these strengths and expertise; A move away from ‘doing to’ and instead facilitating and supporting individuals and communities to work together and with each other.

12 The Future – Sheltered Housing review
Workshops have been held as part of the sheltered housing review. The participants have agreed that an Older Peoples strategy should be developed. A range of stakeholders, including older people’s groups, will be involved in developing the strategy.

13 The Future – Use of technology
There is a plethora of new technologies being developed to change the lives and health of adults. They are playing an important role in shaping this new generation of technology which is developing a range of products with a focus on dignity, independence and choice and to drive forward innovation in 2018/19.
