1 The Models of Care Portal was launched in 2016, there are now over 1000 registered users on the web platform. To encourage continued use of this unique online resource which was developed as part of the programme to support primary care workforce sustainability several new functions are being released: Sustainability Toolkit which features access to the General Practice Workforce Analysis Tool (GP WAT) and a suite of programmes designed to support resilience Improvements to the Collaboration Spaces – secure, safe spaces for private discussion between groups – including easier document sharing and editing, refined forums, better alerts Case Studies and Resources combined to create a pool of dynamic examples of good practice categorised against the 10 High Impact Actions identified in the General Practice Forward View Education – linked to the Community Education Provider Networks with direct access to training opportunities Skills Marketplace to promote and encourage the sharing of individuals’ skills, assets and ideas throughout organisations within the health and care system The Models of Care Portal is available across the NHS to aid learning, share tools and the achievements of health organisations.
