3 Why Introduce a Red Bag Scheme?
Poor communications between Care Homes and the Hospital Residents can remain in Hospital for longer than necessary No standardised paperwork in place No baseline information provided to assess a Care Home resident No medical information on the acute episode provided Documentation gets lost Personal belongings are lost No system in place to track Care Home residents through Hospital.

6 The Red Bag follows the resident throughout their journey from the Care Home to the Ambulance and the Hospital, during their stay and back again.

7 Completing the Red Bag Checklist
The checklist is completed by the Care Home when an ambulance is called. Important to pack all relevant essential items in the Red Bag. Hospital staff will check the Red Bag contents on arrival / admission and ensure it stays with the resident when transferred to a ward. Ward staff will repack the Red Bag and complete the checklist on discharge, ensuring all correct documentation and belongings are included (in particular, the discharge letter and medications). Care Home staff will unpack the Red Bag and check its contents for the final time once the resident has returned to the Care Home.

18 Training Staff Training sessions offered to all Care Home Managers
Toolkit provided for Managers to roll out to their staff based on a “train the trainer” model (sample bag, documentation, posters, power point presentation) Care Home Managers to ensure all staff, especially deputies and senior leads understand the Red Bag process Red Bag to be discussed at regular meetings to maintain awareness.

20 Tracking & Monitoring Care Homes will need to:
Create their own log or booking system to record when the Red Bags are used and returned / cleaned and by whom (helps to identify lost bags) Keep a copy of the Red Bag Checklist in a central folder Supply Luton Council with anonymised data on a monthly basis to monitor use and effectiveness

23 Evaluation The Scheme will be reviewed after 6 x months
Quality Surveys will be developed to capture feedback They will be sent to all Care Homes involved and the Luton and Dunstable Hospital An evaluation report will be produced for consideration by the Luton Red Bag Task and Finish Group.
