The presence of an underlying cardiomyopathy critically determines the subsequent treatment plan. The most challenging decisions are probably in those athletes with ventricular arrhythmias (VAs) and a dysfunctional right ventricle (RV), but no definite arrhythmogenic right ventricular cardiomyopathy criteria fulfilled. Some factors such as the response to exercise or detraining, or a close follow-up after an ablation (in those individuals with suspected idiopathic ventricular tachycardia), might be of diagnostic value.
