2 Introduction to the outcomes-focused practice website
The Care Act 2014 and The Social Services and Wellbeing (Wales) Act 2014 provide a legal framework for improving the wellbeing of people who need care and support, and carers who need support. Promoting wellbeing is a fundamental principle of both pieces of legislation. This site aims to support best practice which promotes a culture of working in an outcome focused way. Welcome to this open access website developed through RiPfA to support a change in practice from traditional care managed service led approach to a culture of working in a person centred outcome focused way. The resources developed within this site have therefore been tested and designed to support practitioners and managers to develop knowledge and skills to work within the legal frameworks for improving the wellbeing of people who may need care and support, including carer’s, starting with the premise of understanding the impact of outcome focused conversations.

4 Interactive triangle of care
research in practice 28/12/2018 Interactive triangle of care The triangle aims to support conversations about outcomes by encouraging practitioners to think about key things that are important to us all The triangle is interactive so can be used before, during or on reflection of a conversation about outcomes. This can therefore be a useful tool to use in supervision as every individual a practitioner is seeing will have an individual triangle unique to them which can change over time. Clicking on each section of the triangle will reveal further information. It is important to view each section as prompts rather than a checklist. The middle section of the triangle has more detail about what core skills and strategies practitioners may wish to explore and develop when having outcome focused conversations. The fact sheets on core skills support practitioners in this.

Each page includes an overview of why outcomes are important from a policy and practice context, case studies and a variety of practice tools to support conversations about outcomes. The audio interviews demonstrate the Core Skills required for outcomes focused conversations including “active listening” and “powerful conversations” and can be used in sections with questions for practitioners to reflect on during /or at the end of the audio tape. Maximum benefit from the audio interviews may be in group situations for fuller discussion and learning. Whereas the individual case studies can be helpful in one to one supervision sessions particularly with less experienced staff.

Supports continual professional development within the KSS Supports continual professional development within the PCF, particularly in the domain for Intervention and Skills Supports practitioners to meet their registration requirements for HCPC – Standards of Proficiency for Social Workers (HCPC 2012) The resources and tools on this website can therefore be used to support continual professional development within the Knowledge and Skills Statement(KSS), The Professional Capabilities Framework (PCF)and registration requirements for the Health and Care Professionals Council (HCPC) Standards of Proficiency for Social Workers(HCPC, 2012)
