2 Internship registration process
To register the internship courses (HSPM 191A & 191B), you have to get an add code from the instructor. To get the add code… Job & Approval from your supervisor University Organization Agreement (UOA) filed on SJS4 (University policy) Internship Application You have to plan ahead of time If you are planning to take the internship course in Spring 2019, you would better plan now or during the winter break. Also, you have about three weeks to add courses after the semester begins. (Check the last day to add courses)

Find the job Confirm with your supervisor Note: The internship application is available on the Department website. -- Students tab -- Forms Submit the signed internship application to the instructor to get an add code Yes Check if your internship site is listed in SJS4 Your internship site needs to submit the New Partner Proposal Form and sign the UOA online Submit a Request for a new site on SJS4 No Submit the signed internship application to the instructor to get an add code Keep checking if your internship site is listed in SJS4 Yes

6 How to check if your internship site is listed in SJS4?
Go to SJS4 website: Click ‘Site’  Type your internship site *Note: If your internship site is listed in SJS4, it is good for five years. If your internship site is not listed in SJS4, you have to submit the “Request for a new site” for your internship site.

7 How to submit the ‘Request for a New Site’ in SJS4?
Go to SJS4 website: If you are logged in, please log out first. Click ‘Home’  Click ‘Student & Faculty request for a new site’  Fill out the form and submit Once you submit the request for a new site for your internship site on SJS4, your supervisor or manager will receive an regarding the ‘New Partner Proposal Form’. They need to fill out the New partner proposal form and submit it online. Once your supervisor or manager submits the New Partner Proposal Form, they will receive another regarding the University Organization Agreement (UOA). They need to follow the directions described in the to sign the UOA online.

8 When you submit the ‘Request for a New Site’, I highly encourage you to sit with your supervisor and work together. If you just submit the request form and inform your supervisor, he or she may forget to submit the new proposal form and sign the UOA. After your supervisor signs the UOA, it would take about business days to be approved by SJS4. You need to keep checking if your internship site is listed in SJS4. If it is not listed in SJS4 within about a week, please contact Center for Community Learning & Leadership at

9 Internship Application
Once you confirm that your internship site is listed in SJS4, you need to work with your supervisor to fill out the internship application. You and your supervisor have to sign the application. (Hand- written signature) Submit the signed internship application to the instructor to get an add code. The Internship application is available on the department website. (Students tab - Forms)

You have to log a placement on SJS4 to submit 1) Release of Liability form and 2) Learning Plan and Participation Guidelines form in SJS4 on the first day of the semester. The instructor will check if you submitted the required forms on SJS4. Your work hours will not count until you submit these forms.

The “student placement guide for internship” is available on the department website. (Students tab – Forms) Go to SJS4 website:  Log in with MYSJSU ID & password  You will be able to see the internship course in SJS4 Please read the “Student placement guide for internship” and follow the directions!!

13 Internship credit Credit/No Credit HSPM 191A: 200 hours & Assignments
HSPM 191B: 300 hours & Assignments Even though you start working before the semester starts, your work hours will count from the first day of the semester.
