1 Antilipemic Agents Lilley Pharmacology Text: Chapter 27
Original Text modified by: Anita A. Kovalsky, R.N., M.N.Ed. Professor of Nursing Original PPT by: Professor Pat Woodbery, ARNP Professor of Nursing

2 Understanding Antilepemic Drugs
You must understand Cholesterol Triglycerides Lipoproteins

3 Lipid Forms Triglycerides = Energy Source
Cholesterol = Makes steroid hormones, cell membranes, bile acids Lipoproteins= transport lipids

4 Cholesterol Balance EAT FOODS RICH IN FATS
Fats: broken down to triglycerides in small intestine Triglycerides: taken into lymphatics and then to liver Liver: lipid metabolism

5 Liver and Cholesterol Lilley pg. 434
VLDL LDL HDL

6 Cholesterol and Coronary Heart Disease Lilley pg. 434
Cholesterol Level >300mg/dl = 3-4X risk then if <200mg/dl

7 Treatment Decisions Lilley pp. 436-437
Total Cholesterol >200mg/dl LDL > mg/dl see patient category page 437

8 Use of Antilipemics Lilley pg. 436
Unsuccessful treatment with diet and exercise for at least 6 months Bile acid sequestrants Niacin HMG-CoA reductase inhibitors

9 How do Antilipemic Drugs Work?
Alter production, Alter metabolism, Assist in removal of lipids and lipoproteins

10 Who takes Antilipemic Drugs?
Clients with elevated cholesterol: When 6 months of life style changes and diet changes do not lower cholesterol Those with coronary artery disease (CAD) Those with family history Those with other disease processes: DM, HTN, SMOKERS)

11 Prototype Drug: Simvastatin (Zocor) (Trade name in parentheses) Lilley pg. 439
STATIN DRUG Inhibits an enzyme (hydroxymethylglutaryl-coenzyme A reductase) required for hepatic synthesis of cholesterol

12 Nursing Actions Check for lowering of cholesterol Take in the evening
Observe for side effects GI problems Abnormal liver enzymes Muscle pains

13 Client Teaching Prevention of stroke, heart attack, peripheral artery disease Continue on low fat diet Increase sensitivity to light Take in the evening Liver Function tests are recommended at first 6-12 weeks then every 6-12 months

14 The END
